Book A Ride

1. Book your ride at least 24-48 hours before.

2. To schedule your service, have the following information ready:

​-  Your date of birth, first and last name, and phone number.

-  Medicare or Medicaid ID number.

-  The pickup address of where we will be picking you up.

-  Your doctor’s or facility’s name, address, and telephone number.

-  The date and time of your medical appointment.
